ISSUANCE OF EXCAVATION AND GROUND PREPARATION PERMIT

REQUIREMENTS

  • Duly accomplished Excavation Permit form
  • Barangay Clearance (1 Original, 3 Photocopies)
  • Homeowner’s Association Clearance, if any (1 Original, 3 Photocopies)
  • Certified True Copy of TCT/OCT (1 Original, 3 Photocopies)
  • Certified True Copy of Tax Declaration and Photocopy of Tax Receipt (1 Original, 3 Photocopies)
  • Vicinity Map/ Lot Plan/ Location Plan
  • Four (4) sets of Plans and Documents signed and sealed over the printed name of the duly licensed and registered professionals.
    1. Civil/ Structural Plan
    2. Bill of materials and Cost Estimates
    3. Specifications
    4. Construction Schedule with exact date
  • Photocopy of PRC ID and PTR of Duly Registered Professionals who signed the plans and documents, with original signed and sealed
  • DPWH Clearance for projects along National Road within local jurisdiction where the project is located
  • DOLE Approved – Health and Safety Program For Large Scale Projects 
    1. Request Letter to Mayor’s Office (Request for Excavation)
    2. Endorsement from Mayor’s Office

STEPS

  1. Submit the complete requirements
  2. Accompany the Engineering Staff in the inspection of the site/project location
  3. Proceed to the Office of the Municipal Treasurer for payment
  4. Present the Official Receipt (OR) to the Building Official
