ISSUANCE OF SIGN PERMIT

REQUIREMENTS

  • Duly Accomplished Sign Permit Form
  • Request Letter
  • Endorsement from Mayor’s Office (Request for Sign installation)
  • Barangay Clearance (1 Original, 3 Photocopies)
  • Community Tax Certificate
  • Four (4) sets of Plans and Documents signed and sealed over the printed name of the duly licensed and registered professionals. 
    • Location/ Site Plan
    • Plans indicating the following
      1. Complete dimension of the signage
      2. Number of signage to be displayed
      3. Materials used for Construction
    • Structural Plan, if necessary 
    • Electrical Plan, if necessary
  • 3 Photocopies of PRC ID and PTR of Duly Registered Professionals who signed the plans and documents, with original signed and sealed
  • DPWH Clearance for projects along National Road within local jurisdiction where the project is sited

STEPS

  1. Submit the complete requirements
  2. Accompany the Engineering Staff in the inspection of the site for installation
  3. Proceed to the Office of the Municipal Treasurer for corresponding fee
  4. Present the Official Receipt (OR) to the Building Official of the Municipal Engineering Office
