REQUIERMENTS

• Photocopy of OSCA ID with 3 Signature
• Application Form ( from OSCA Office )
• Certificate of Indigency ( from Barangay )
• Medical Certificate ( If needed )

STEPS

1. IDENTIFICATION – 60 Years of Age and above. Identify Indigent Senior Citizens, Economic Status of Senior No Pension from GSIS, SSS, PVAO, AFPMBAI or any other Insurance company. No Permanent Income, No Regular Support from family or relatives for his/her basic needs.
2. ASSESSMENT & VALIDATION –
   • Interview by Region Potential Beneficiaries.
   • Social Pension General intake Sheet (GIS)
   • Photocopy of OSCA ID with 3 Signature
3. INCLUSION TO SOCIAL PENSION PAYOUT – The Focal Officer Shall endorsed the approved final list from the DSWD Central Office of beneficiaries to the LOCAL SOCIAL WELFARE AND DEVELOPMENT OFFICE (LSWDO) for reference
4. NOTIFICATION – The OSCA Head will immediately inform the indigent senior citizen and/or his/her family through a written notification on his/her approval as Social Pension (SP) Beneficiary based on the approved list provided by the Focal Officer. The OSCA Head may also coordinate with the respective Senior Citizen Presidents to assist in the dissemination of notice to the qualified beneficiaries.
