REQUIREMENTS

• Duly accomplished COD
• Negative Result from PSA (1yr. And above)
• Affidavit for Delayed Registration
• Burial Certificate

STEPS

1. Submit the duly accomplished COD with the requirements in the MCR Office
2. The client will sign the Affidavit for Delayed Registration at the back of COD
3. Pay the corresponding fee in the Treasurer’s Office
4. Submit the Receipt to the MCR Office
5. Claim the registered COD after the 10-day posting period
