• Certificate from CENRO
• Affidavit of Ownership
• Sworn Statement Declaring the True Market Value of the Property
• Certification from Barangay
• Assessor Inspection Report

STEPS

1. Submit the requirements
2. Proceed to the Municipal Treasurer’s Office
3. Submit Official Receipt to the Municipal Assessor’s Office
4. Field Work
5. Transmit the documents to Provincial Assessor’s Office  for approval
6. Encoding and e-approval
7. Await for the release of copy
