REQUIREMENTS

• Duly accomplished COM
• Negative Result from PSA (1yr. And above)
• Affidavit for Delayed Registration

STEPS

1. Submit the duly accomplished COM with the requirement/s in the MCR Office
2. The client will sign the Affidavit for Delayed Registration at the back of the COM
3. Proceed to the Treasurer’s Office for the corresponding fee
4. Submit the Receipt to the MCR Office
