REQUIREMENTS

• PSA Copy of COLB (if registered) / Duly Accomplished COLB (if not yet registered)
• Affidavit of Paternity
• Joint Affidavit of Legitimation
• PSA/CTC Copy of Certificate of Death
• CENOMAR
• PSA/CTC Copy of Certificate of Marriage

STEPS

1. The client should submit the requirements to the MCR office
2. Proceed to the Treasurer’s Office for the corresponding fee
3. Claim the Legitimated COLB after 2 working days
