For Client need for Social Case Study Report

• Barangay Indigency Certificate
• Medical Abstract or Medical Certificate
• Hospitalization bill, Laboratories request and other medical request
• Endorsement from other partner agency (if available)

STEPS

1. Client or relative submit the requirements at the MSWDO
2. Intake interview and Initial assessment
3. Prepare the Summary Case Study Report
4. Checking and approval of the SCSR
5. Release SCSR

REQUIREMENTS

For referral to NCMH

• Barangay Indigency
• ID of client
• Referral from doctor (if available)

STEPS

1. The client’s family ask for referral at the MSWDO
2. Intake interview and initial assessment to the client
3. Prepare the Case Study Report and Referral to NCMH
4. Checking and Approval of CSR and Referral
5. Release of Referral
