REQUIREMENTS

• Duly accomplished COLB
• Affidavit to Use the Surname of the Father (AUSF) executed by the child’s mothe
• Community Tax Certificate (CEDULA) or any government-issued ID of both parents

STEPS

1. Submit the duly accomplished COLB in the Office of the MCR
2. The mother of the child will execute the AUSF
3. Submit the duly accomplished COLB and AUSF with signatures
4. Procced to the Treasurer’s Office for the corresponding fee
5. Submit the Receipt to the MCR Office
6. Claim the registered COLB
