• Statement of Account (SOA) of previous year
• Barangay Clearance/ Market Clearance for Market
• Sworn Declaration of Gross Sales (notarized)/ BIR Returns of previous year
• Community Tax Certificate

STEPS

1. Submit the accomplished form with complete requirements and assessment of fees and taxes
2. Proceed to the Treasure’s Office for the corresponding fees
3. Await for the release of Business Permit with Plate
