REQIUREMENTS

If Registered:

Registered Death Certificate

If Not yet Registered:

Death Certificate must be signed at the point of origin

For Exhumation Permit:

Exhumation of cadaver must follow the required minimum number of years burial set by the Code of Sanitation of the Philippines before exhumation

STEPS

1. Provide the necessary documents required and present it to the MHO- Sanitation Section
2. Proceed to the Municipal Treasurer’s Office for the corresponding fee
3. Issuance of the Permit
