REQUIREMENTS

• Patients who were advised to undergo tooth extraction must have medical clearance or certification of no history of medical complications such as hypertension, heart disease, allergies, diabetes, and exposure to any viruses such as Covid-19

STEPS

1. For Dental consultation and appointment, please like Facebook page of Municipal Health Office – Angono, Rizal and send message. (Teledentistry)
2. Patient must secure certification from their respective Barangay Health Station stating that the patient is not included in the PUI (Suspected/ Probable list of Covid-19 and other communicable diseases
3. Patient must present the acquired Health Clearance prior undergoing to Dental Check up and assessment
4. Patient/s with dental appointment must go to the Municipal Health Office OPD section for assessment and taking vital signs
5. Patient will pay the necessary fee in the Municipal Treasury
6. Patient with accomplished assessment form and receipt from treasury can now be given the appropriate dental service
7. After dental extraction, patient is prescribed necessary medicine
